For value received, receipt of which is acknowledged, I hereby give Sysco Corporation, its subsidiaries, affiliates or divisions (collectively referred to as "Sysco") the absolute right and permission to copyright, publish, or use my likeness, photograph, voice, or any video or other images in which I may be included in whole or in part, in composite or distorted character or form (collectively, the "Works"), in conjunction with my own or fictitious name, or reproductions thereof in color or otherwise, in any and all media now or hereafter known, for illustration, promotion, art, advertising and trade, or any other purpose whatsoever. I hereby release and discharge Sysco from any and all claims and demands arising out of or related to the use of the Works, including without limitation any and all claims for copyright infringement, moral rights, libel, or invasion of privacy.
I hereby waive any right that I may have to inspect and/or approve the finished product or the advertising copy that may be used in connection with the Works, or the purpose for which Sysco uses the Works.
I acknowledge and agree that the Sysco: (a) has the right to supervise and control directly the creation of the Works; (b) has the exclusive right to use (or not use) the Works, or any portion thereof, as it sees fit; (c) may alter the Works, or add to or combine the Works with any other photographs, images, video, languages, voice overs, drawings, graphics, or other content in its sole discretion; (d) may display the Works in any media, including without limitation print and electronic media and any and all media yet to be created; and (e) may digitize the Works for future distribution in its sole discretion.
Sysco may sell, assign, license, or otherwise transfer all rights granted to it hereunder. This authorization and release shall also extend and inure to the benefit of the successors, legal representatives, licensees, and assigns of Sysco and its officers, directors, employees, servants, and agents.
I am of full age and have the right to contract in my own name. I have read the foregoing and fully understand the contents thereof. This release shall be binding upon me and my heirs, legal representatives, and assigns.
I hereby waive all residual rights or claims, monetary or otherwise, that arise because of any additional use of the Works at any future time.
This agreement shall be governed by the laws of the state of Texas. The federal and state courts located in Harris County shall have exclusive jurisdiction to hear claims arising from or relating to the terms and subject matter of this Photo/Video Release.
If under age 18, signature of parent or guardian is required.
